Наряду с последним в формировании социальной изоляции участвует активно воспалительная реакция, но она изучена при социальной изоляции лишь в единичных публикациях по изменению в системном кровотоке классических маркеров воспаления &amp;ndash; С-реактивного белка и IL-6, а среди пациентов с возрастной макулярной дегенерацией вообще не рассматривалась. Цель исследования: Изучение особенностей воспалительного процесса у пациентов с социальной изоляцией и возрастной макулярной дегенерацией. Материалы и методы: Обследовано 92 пациента 60-74 лет с социальной изоляцией и возрастной макулярной дегенерацией, а также 88 пациентов того же возраста с социальной изоляцией без возрастной макулярной дегенерации. Выявление социальной изоляции выполнено по модифицированному индексу социальной изоляции. Возрастная макулярная дегенерация диагностировалась на основе критериев классификации Age-Related Eye Disease Study и по результатам многопланового офтальмологического обследования. Воспаление оценивалось по содержанию в периферической крови С-реактивного белка, интерлейкинов и хемокинов. Результаты: Среди пациентов пожилого возраста с социальной изоляцией и возрастной макулярной дегенерацией по сравнению с представителями, имеющими только социальную изоляцию, установлены статистически значимо более высокие уровни всех изученных биомаркеров воспаления. Особенно существенно у пациентов с социальной изоляцией и возрастной макулярной дегенерацией выше было содержание в крови хемокинов GRO(&amp;alpha;)/CXCL1 и MIP-1(&amp;alpha;)/CCL3, экспрессия которых составила 18,3&amp;plusmn;1,4 пг/мл и 3,8&amp;plusmn;0,7 пг/мл соответственно против 6,4&amp;plusmn;0,9 пг/мл и 1,7&amp;plusmn;0,3 пг/мл в группе лиц с социальной изоляцией. У пациентов с социальной изоляцией и возрастной макулярной дегенерацией статистически значимо выше оказалось содержание в крови IL-6, достигшее 50,2&amp;plusmn;2,2 пг/мл против 24,9&amp;plusmn;1,4 пг/мл в группе лиц с социальной изоляцией (P&amp;lt;0,001). Следует также отметить повышенный уровень IL-7, повысившегося до 24,5&amp;plusmn;1,7 пг/мл против 14,3&amp;plusmn;0,9 пг/мл соответственно (P&amp;lt;0,01). Заключение: Сочетание социальной изоляции и возрастной макулярной дегенерации в пожилом возрасте сопровождается более высоким содержанием всех маркеров воспаления и, прежде всего, цитокинов GRO(&amp;alpha;)/CXCL1, MIP-1(&amp;alpha;)/CCL3 и IL-6, которые следует применять в качестве маркеров социальной изоляции при возрастной макулярной дегенерации</p></abstract><trans-abstract xml:lang="en"><p>Background: The priority problem of modern gerontology and geriatrics is currently social isolation, which represents an objective individual condition and reflects the absence of social relationships or the rarity of social contacts. Together with the latter, an inflammatory response is actively involved in the formation of social isolation, but it has only been studied in social isolation in isolated publications on changes in the systemic blood flow of classic markers of inflammation &amp;ndash; C-reactive protein and IL-6 &amp;ndash; and has not been considered at all in patients with age-related macular degeneration. The aim of the study: To study the features of the inflammatory process in patients with social isolation and age-related macular degeneration. Materials and methods: 92 60-74 year-old patients with social isolation and age-related macular degeneration, as well as 88 patients of the same age with social isolation without age-related macular degeneration were examined. The identification of social isolation was performed using a modified index of social isolation. Age-related macular degeneration was diagnosed based on the classification criteria of the Age-Related Eye Disease Study and based on the results of a multidisciplinary ophthalmological examination. Inflammation was assessed by the content of C-reactive protein, interleukins and chemokines in peripheral blood. Results: Statistically significantly higher levels of all studied biomarkers of inflammation were found in elderly patients with social isolation and age-related macular degeneration compared with representatives with only social isolation. Especially significantly in patients with social isolation and age-related macular degeneration, the blood content of the chemokines GRO(&amp;alpha;)/CXCL1 and MIP-1(&amp;alpha;)/CCL3 was higher, the expression of which was 18.3&amp;plusmn;1.4 pg/ml and 3.8&amp;plusmn;0.7 pg/ml, respectively, versus 6.4&amp;plusmn;0.9 pg/ml and 1.7&amp;plusmn;0.3 pg/ml in the group of people with social isolation. In patients with social isolation and age-related macular degeneration, the blood content of IL-6 was significantly higher, reaching 50.2&amp;plusmn;2.2 pg/ml versus 24.9&amp;plusmn;1.4 pg/ml in the group of people with social isolation (P&amp;lt;0.001). It should also be noted that the increased level of IL-7 increased to 24.5&amp;plusmn;1.7 pg/ml versus 14.3&amp;plusmn;0.9 pg/ml, respectively (P&amp;lt;0.01).
